In accordance with the provisions of the Federal Law on Protection of Personal Data in Possession of Individuals (LFPDPPP), Investigaciones Medicas del Potosi, S.A. de C.V. which commercial name is Hospital de Especialidades Medicas de la Salud, with address at Avenida Benito Juárez # 1210 Fracc. Valle Dorado Z.C. 78399 in the municipality of San Luis Potosi, S.L.P., informs you of the following:
That the personal data provided by you to this Hospital, including sensitive, patrimonial or financial information, by means of requests, declarations, electronic means, printed in all its modalities, videos and other analogous concepts, were collected for the provision of the services provided to you by this Hospital, personal data that will be used and treated exclusively for the purposes related to your medical and hospital care, as well as for the effective fulfillment of all and each of the obligations derived from the relationship between the various professional, technical and auxiliary health services that intervene in said services, such as treating physicians, interconsultants, support personnel to them, companies that provide subrogated services , and affiliated subsidiaries or companies that are part of this Hospital.
This Hospital also informs you that in order to limit the use and disclosure of such personal data, we will maintain internal administrative, technical, physical and confidentiality security policies and procedures.
If it is necessary to transfer such personal data for the payment of the services rendered by third parties outside of this institution, with whom medical coverage services have been contracted, they may be among them in a non-limiting manner. Insurers, agreements with state, municipal or decentralized entities, banks, health system administrators, and other similar institutions, this Hospital informs you that your personal data will be treated in the terms provided in the Article 37 of the Federal Law of Protection of Personal Data in Possession of Private Parties, assuming the third party receiver, the corresponding inherent obligations, within which is to safeguard the privacy of the same.
In the same way, your personal data, even sensitive ones, may be shared with public notaries, and / or Hospital Lawyers, on the occasion of the legal acts that may be generated as a result of said provision of services.

The owner of personal data may revoke the consent with the acceptance of this. Said revocation of the consent that is granted by electronic means, shall be done observing the following procedure:
Send an email and / or in writing to the Responsible area designated in this Notice, through which said requests will be addressed:
* The full name of the Owner, address and email;
* The reason for your request;
* The arguments that support your request or application;
* Attach an official document that proves your identity and shows that you are who you say you are.
The HOSPITAL DE ESPECIALIDADES MEDICAS DE LA SALUD in accordance with Art. 32 of the Law on the matter, will notify the Holder, within a maximum period of twenty days, counted from the date on which the request was received on the exercise of the ARCO rights, the resolution adopted, to the effect that, if it is appropriate, it becomes effective within 15 days after the date on which the resolution will be communicated, by means of a letter or via e-mail where it contains that all the acts have been executed tending not to treat the personal data of the Owner.
If you do not express your opposition for your personal data to be transferred, it will be understood that you have given your consent for it, such consent being valid for all medical and hospital services that from now on lend to you you this Hospital.
This Hospital reserves the right to make modifications or updates to this Privacy Notice at any time, to comply with new legislative or jurisprudential provisions, internal policies and / or new requirements for the provision of medical and hospital services provided by the Hospital. This information will be available on the website www.hems.com.mx and / or through communications placed in the premises occupied by this Hospital.
The parties express that this notice will be governed by the legal provisions, applicable in the Mexican Republic in particular, by the provisions of the Federal Law on Personal Data in Possession of Individuals (LFPDPPP) and its regulations.
In the event that there is a dispute or controversy, derived from the interpretation, execution or fulfillment of the notice or any of the documents derived from it, or that are related to it, the parties will amicably seek to reach to an agreement within a term of thirty (30) calendar days, counted from the date on which any difference arises and the counterparty is notified in writing about that event.
In case the parties do not reach an agreement, both agree to submit all disagreements arising from this Privacy Notice or any of the documents derived from it or that are related to it or to those that definitively are part of the jurisdiction of the courts of San Luis Potosi, expressly renouncing any other jurisdiction that for reasons of their present or future domiciles may correspond to them.
